CheckForm

CheckForm is an online service provided by the Central Election Commission of Ukraine. It is operated by the Data-Driven Governance Project, which is responsible for the system's management and maintenance.

The purpose of CheckForm is to allow voters to verify their information on the voter list before an election. This helps to ensure that all eligible voters are registered to vote and that their information is accurate.

To use CheckForm, voters simply need to enter their personal information, including their name, date of birth, and address. The system will then search the voter list to find their registration information.

If the voter's information is found on the list, they will be shown a summary of their details, including their polling station location and voter identification number. If there are any discrepancies in the information, the voter can contact their local election office to make corrections.
